I have moved hosting providers and in making sure all is working i have found that you are blocking notification emails from the form, as below:
'This email address is blocked by Jotform, please contact us.'
Can you please let me know why this is and if it can be changed?

I have removed your email address from blocked list. You should be able to receive notifications now.
When the email has not been successfully delivered and the reason reported back to us is that the email was bounced, that email appears on the blocked list.
This sometimes happen when you're email service provider rejected our email at some point or when they're not available to received message from us.

Thanks for unblocking the email.
I was using Office small business and was too slow moving providers before they closed my account!
Still, its all working now :)
