    Hi robbfahrion,

To allow users to submit multiple answers you can use three different field types:-

Drop Down Menu: This allows you to insert multiple answers and your form user to extend the menu and choose from the list.

Radio Button: Allows you to submit multiple answers, however only allows your form user to choose ONE from the list.

Checkbox: Allows you to submit multiple answers, and the form user can choose as many answers in that list as they want.

    Hope this helps, Greg.

You can use a check box for that. Go to Form Tools > Check box .
