Are you completing this form on behalf of a church (yes - leads to additional questions and no - leads somewhere else)

You can create an conditions here in JotForm. Please check this demo form: https://form.jotform.com/70036479562965.
Here's a guide on how I created that demo form:
I inserted 3 "Radio Button" fields in the form.
After that, you need to create a condition wherein if Question 1 is equal to "Yes" then the Question 2 will appear. You can use the "Show / Hide Field" condition for this. You may refer to this user guide: https://www.jotform.com/help/316-How-to-Show-or-Hide-Fields-Base-on-User-s-Answer.
