Edit form question

I am attempting to get full name and birthdate to line up horizontally for at least 4 passengers for the purposes of a cruise or needing plane reservations.  I can get them to align horizontally only once on the form.  Please see my work in progress - URL below.  I look forward to hearing from you.

I have edited the form for you and fixed it. You just have to right click on the field the click Move to a new line.
