I checked your form and I was able to replicate the issue you are reporting. When I clicked submit, even if the Terms and Condition field is selected, the required error message is still displayed.
Please try to the following steps to fix the issue.
1. Open the Widget Wizard.
2. Click Update Widget without changing anything.
The following steps above will refresh the setup of the widget that will resolve the issue.
