I am looking to create an online Seller Property Disclosure Statement. I am a Realtor and would like my customers to be able to visit my site and fill out the form, hit a button and have the for emailed directly to me. The form we have is 10 pages and is part of the PA Property Disclosure Law.

I can send you a copy of the form if you like.

@jeffreyhoguerealtor, login to your account, and click on Create Form:

Then, you will see the "Request a Form" option, simply click on the request button:

There you will be able to give all the details(type of fields, amount of fields, etc) of the type of form  you want to.
