Why does my JotForm appear differently on Firefox & Chrome, than on Safari?

After clearing all history, the cache and refreshing all 3 of my browsers, my JotForm appears differently on Firefox & Chrome, than it does on Safari. Only on Safari are there no issues… it is perfect - just the way it was designed.

When I drop my URL http://johntoddjostens.com/QuoteRequest.html in the 
search bar of Firefox & Chrome - for the first (split) second I believe I'm viewing 
the proper fields visible to me only on Safari… then the images switch and becomes 
visible with a totally different format allowing me to only get as far as shown in the screenshots. 


Does anyone have a solution to this scenario?

It looks like your form is not being responsive in mobile. Can you please try checking the option to make your form mobile-responsive from the Form Designer?
