Can I get help moving my logo on my form

Hi. In an effort to not mess anything up, I am hoping I can get help moving my logo above the text on my questionnaire. Please and thank you!

I am not really sure where you wanted to place the logo, but based on my assumption, I created screencast explaining how you can move the entire header blog anywhere in the form or alternatively, if you want to place the logo above the header text then you can remove image from header and create a new image using image field and place if above the header
