I am the contact for the users of this website and this is the first issue that has arisen. I am sure it is something he has done but would to respond with a 'knowledgeable' answer!

I sent a test submission and your form is working properly. I also checked your email logs. There seems to be no issue.
I couldn't check the issue on the web page that you have embedded your form since I don't have login credentials. If you want us to further investigate the issue on your web site please provide us login details.
If the issue happens again could you send us the details of the machine and browser that the issue happened. With this information we can investigate the issue further.
