Can you have a table checkbox?

Please see below picture of what I would ideally like to do or any other better ideas?

We do not have the means to create the exact structure as depicted in your image, but you could create a similar form by using our ordinary Matrix field as your table:

To have your Matrix field collect answers through Check Boxes, simply click on its Input Type, and select Check Box:

Alternatively, you can use our regular Check Box field as well, through you will need to write the entire text of your options (such as the course name, duration, and price) as a label of each of the options inside the Check Box field.
