I tested your form on an iPad and did not see any of the jumping to the top or freezing. The only time the form jumped is when attempting to submit an incomplete form. The position then jumped to whatever required field was not fully filled.
I also sent a submission through the form and did not need to press "Go" after pressing submit. Try using a fresh install of Google Chrome or FireFox browser on the ipad. Either of those browsers should work better than Safari, especially if you are using an older version.
