I made an edit and my form disappeared. The title changed to "Title Me" as well. You can see the form in the revision history, but it doesn't show up under my forms to edit, nor can you see it on my website.

I am not sure what exactly happened, but I saw that your form is blank and that title of the form is "Title Me". Did you tried to revert you form to previous version? If not please do. If that does not resolve your issue, please let us know. With your permission we can also try to revert the form from our end. And if we are also unable to do it we will escalate this issue to our developers.
Also can you tell us what exactly did you changed on your form?

Hi, thanks for the response. I tried to revert the form, and that's when it went blank. I just tried to revert it again, and it still is the same. Yes, you have permission to revert it. Thanks!

I was able to replicate mentioned issue. It seems that when form is reverted, instead of previous version, all fields are removed from the form.
I will raise this issue to our developers and we will inform you via this thread once this issue is resolved.
Also please do not use revision history option until this issue is resolved.

This issue is now resolved. All fields are shown again on your form. If needed you can revert your form to correct version.
