Forms doesnt show on ipad

dear sir or madam, i tried our first form on ipad but it doesnt show. Not in the preview mode, nor as a real form in either safari or chrome. The device is an airpad air. The form works fine on android phoen or pc.

I also could not view the form on my desktop - chrome browser. I resaved your form in the form builder and it fixes the problem.
