My form has been disabled. Can someone please help me enable it again?

    I am creating a form for my school to use for students to sign up for Advanced Placement tests from the first of February to mid-April.  When I try to preview the form to check the conditional logic, it says that the form has been disabled.  I've been using JotForm as a free user for years and have never had this issue before.  We will be upgrading the account to a paid account so that we can actually use the form, as we will need to receive payments for several hundred students.  Could you please let me know what caused my form to be disabled so that I can avoid whatever triggered it in the future?  Could you also please re-enable the form so that I can make sure it's working properly and so that I can add it to our school website before February 1?  Thank you!

    We apologize for the inconvenience caused. The Phishing detector had suspended the form due to suspected phishing form. However, after manually reviewing your forms and account, I found that it was a mistake.

    I have now re-activated your account and forms. You can login to check the updated status at https://www.jotform.com/login

    Let us know if issue still remains.
