I have inspected your form http://www.jotform.us/form/70005967845159 inside of V4 Form Builder and I was able to replicate mentioned issue. However on your live form apostrophes are shown correctly.
I will escalate this issue to our developers and we will inform you via this thread once this issue is resolved.
For now you can switch back to Version 3 of our form builder if you want to see apostrophes correctly inside of Form Builder as well.
You can switch to Version 3 on settings page of your account.

Thank you for getting that fixed up. In your image though you are referencing a different form. That form displays correctly. Its the other form that does not.

Sorry about that. I found your other form http://www.jotform.us/form/62806631178156 and I saw mentioned issue on live form as well.
To resolve this issue, please switch your Form Builder to Version 3 and then open field properties for both fields and change encoded value ' with an apostrophe.

I am having a similar issue with Version 4. In my "Letter to Springfield" form, I inserted a text field and inserted placeholder text into the Default Value field of the advanced options. Any apostrophe turned into ' on the live form. I switched to Version 3 and that fixed it.
Please let me know if this can be fixed for Version 4 of the Default Value field.
