Is it possible to have question disappear from a form once checked.  Say you give them 1-4 choice, i would like to 1 to disappear after they choose it

Yes, it is possible to hide questions as soon as they are filled, here's a sample condition:

But you can only do this with the entire question, options in a field cannot be affected with conditions.
