the google docs integration works for the first couple of entries, but then fields are missing, but only from the google docs form. fields are missing or misplaced.

I have made a test with a sample form and have sent 5 submissions and all of them will update on my spreadsheet.
I also cloned your form and I am able to see ALL of the fields. Probably you think the fields are missing because the checkboxes are sent to a single field called "I offered.." or "I was able to" .
If you still think the issue persist, please re-integrate google.

I have the exact same issue. My form, http://form.jotform.us/form/21848645631156, properly populates the jotform.com results spreadsheet but leaves many fields blank on google docs. I'll try reintegrating and also creating a clone, but I still suspect other users will experience the same problem.
