I followed the instructions on adding a form to Facebook and it does not seem to populate.
When I click on the tab where the form is it takes me to the setup page where you paster to form code.
Why wont it bring me to the form?

Should you be able to view the form while you are logged in?
When I am logged out and view the URL i see the form. When I am logged in it takes me the the page where you embed the code.

Did you follow the instructions my colleague pasted for you? also, if you still have problems could you please provide with a snapshot?

Yes, I followed the instruction to the T. Even watched the video. The first image is when I am logged in. I follow all the steps, go back to my page, click the tab to bring me to the form and this appears.
The second image is of what I get when I am not logged in. I can see the form.

Is the 2nd image correct? If so, then the page works correctly :). Please note that when you are logged in as the Page Admin/Owner, you will not be able to view it as a form, except it's codes (clicking preview should help as well). That's how it works with Static Iframe. Please use different account when viewing your form or you can logout instead.
