Thanks for letting us know about this problem you are having with your form not working.
I've taken a look at your provided link to your embedded form which is visible on my end.
It might have been a temporary problem and if so then we do apologize about any inconveniences.
Are you still having this problem?
If so then what isn't working?
Was the form not able to submit or did you mean it wasn't displayed on your website perhaps?

It was on my website but the form#31546291079862 was not in my "My Forms" page when I log in. The other two were. All submissions that were made for job applications are gone because the form wasn't working. Apparently it was deleted on your end because today was the first time I logged in to your site.
Is there any way you can recover those job applications that were submitted? We are conducting interviews and we never received the applications that were submitted online over the past several weeks. They did not go to our email because again, the form was gone.

I was not able to find your form with ID 31546291079862 on webpage you provided. Form with ID 70224440213137 is embedded on that page and that form is created on your account holbein2.
To view your submissions from your form with ID 31546291079862 please login to your account holbein1.
