Form Separator Widget not functioning

    Widget not appearing, unformatted source code appears instead.

    I have cloned your form and I was able to replicate same issue on my cloned form as well. 

    On my cloned form I was able to resolve this issue be deleting Form Separator Widget and adding new one. Please try the same on your form.
