How can I add a block for personal/professional references on a job application?

What is the best block to use in order to have applicants type in the name, address, phone and email address of 3 personal references on a job application? I am trying to consolidate into one question, rather than 3 separate text boxes.

If you would like to collect the data in a single field, I would recommend using a configurable list widget:

http://widgets.jotform.com/widget/configurable_list

This will allow you to create a set of varying types of fields.  While the users are filling out the form, they can add more sets as needed.
