How to activate form?

Where do I go to change reactivate the form? 

If I understand you correctly, you want to activate your form. If you disabled your form, see the image below how to enable it:

If you put your form to Archive folder, see the image below to Unarchive the form:

Also, you can try Enabling the form in the Preferences setting. Here's how:

1. Open your form and click the Preferences button in the Form Builder.

2. In the General tab, you can also Enabled or Disable your Form Status.
