I am having issue with my account. When I try to view my forms it shows the form is disable but when I review the status of the form it shows enable. Also, when I try to log in to my account it shows the message "AUTOSUSPENDED". What is the issue? Can you please fix this.

I checked your forms and they look to be within our terms of use. I believe the Skype ID field in your forms is what set off our phishing detector. I activated your account and your forms are usable once again.
