Form showing in Chrome on one computer but not the other

Previous question missed these details.  Chrome version: Version 56.0.2924.76 (64-bit).  

I checked your website http://saludhs.com/employment and the stand alone version of your form https://www.jotform.com/61794368406162 - The form showed up properly on Chrome Version 55.0.2883.87 m (64-bit).

Here's how it looked like from my perspective:

Knowing that it worked on my end, worked on yours, except for another computer, we can safely deduce that the form works as intended and the issue is isolated to that browser alone. Try clearing the cache and cookies of the browser where you failed to see the form to see if that will help.
