I am a mental health provider and would like to have my clients fill out forms on my website (which is hosted by therapysites.com). If they submit all of their information, will it follow HIPPA compliance?
Also, when it is submitted to me, what format will I receive it in? I need it to be in a format that I can save to their chart.

Currently Jotform does not hold a HIPAA Compliance Certificate, nevertheless you can use Jotform in a HIPAA Compliant way.

When form is submitted you will receive email Notification with all data that user entered. You can also attache PDF report of submission to email Notification.
You can also view submitted data on submission page of your form and you can create different type of reports.

I had seen info on the website about HIPPA compliance, but it was a year old so I wasn’t sure if anything had changed.I’m also trying to figure out if I can upload my current PDF forms (either from my hard drive or from my website). I pasted the URL into the upload, but it didn’t work. Here is the URL…can you please tell me if this is possible.
