I've just checked the URLs above and your form seems to be working fine. Have you tried clearing your borwser's cache or tried on a different browsers?

I did not say that the form was not working - - I need to modify it. Where is it in the My Forms area? How can I save it with a specific name?? I want more than one form – is that possible?

Where is it in the My Forms area?

Login to your JotForm account, click on ''My Forms'' page and you will be able to edit your existing form. All your forms are listed under My Forms page.

How can I save it with a specific name?

While you are on your Form Builder (where you edit your form), click on the title in order to change it.

I want more than one form – is that possible?

Yes, you can create more than one form. There is no limit.

The form is there now – but it was missing earlier. I had to import the form from the URL, then modify & save. Thanks for your help. I hope the form stays on the site from now on.
