I can no longer remove any of the folder tags from any forms. I used to be able to click on the "X" of the folder name to remove a folder tag but it doesn't work and if there are 2 folder tags, when trying to remove one tag, it duplicates the second folder every time I click.

I was able to see the problem also when I checked on the My Forms list. I could not remove the form in Folder name by clicking the 'x' icon.
The workaround that you do is like this...
Click on the APPLY button while the Folder name was unchecked. This will save the option and will remove the forms within the folders.

When you do this the page will not be reloaded nor the folder refreshed, could you please try reloading the page after you un-check a form from a folder?
This should work as per my tests. Also, if the problem persists, please let us know which forms are you trying to remove from which folder, we will be glad to test directly on your end in order to find if this is an isolated issue with your account.
