Why I am getting "Form not found" Error?

Whenever I try to view any of my forms on my account, before or after the 4.0 update, I get a "Form not found" error. Is there an issue with my account or just a bug in the system?

I'm researching other answers, is it because of something I added in my latest form that disabled my account, or is it something else that's the matter?

I have checked your account and I have seen that your account has been auto-suspended due to phishing as you have requested Reddit username information from users as the following screenshot:

This situation violated our Term Of Use. You are not allowed to receive login information in your account. So, your account has been suspended and your forms become unreachable. It is not related to version JotForm 4.0.

I had to delete this forbidden field from your form and re-activated your account again.
