With which form you are having this issue with?
What is you JotForm account username or the email address you have used in your account profile? I am unable to find any details with your email address asif_sheikh@live.com. Please let us know any of your form ID / form URL or web page URL where you have embedded your form and we will take a look.

I've tested your form named "Mohawk Safety Incident Report" and I have submitted the form successfully as you can see the screenshot below:
All in all, I could not produce any problem regarding your form. The issue seems to be solved.
Did you embed your form into a website? If so, please give us the website URL in order to let us investigate the issue further.
