Patient Agreement Form

Hello,

I created a new patient registration form with JotForm.  On shopify, is there a way to require customers to sign that form while they are signing up for an account on my website?

You may capture the basic info such as name, email, phone number, address and other info while creating the account on your website and then send this info to the JotForm when the signing process is completed.
