Why is my form disabled?

    Why does my form say "Form not Found. This form is disabled." When I try to preview it?  I have switched form status to "enabled" in the settings but still no luck. Please help ASAP! 

    I added the webpage where the form is embedded but it doesn't show up

    I see that the account is auto-suspended by our anti-phishing system which is strict to scan each form to calculate the probability of phishing activities. It is not allowed to create login forms or collect sensitive information like Credit card numbers, etc through JotForm. Such forms are considered as phishing forms and associated accounts are automatically suspended. Please refer to our Terms of usage for more information on phishing.

    I have checked your forms and see that they are normal and clean. I have activated your account and you should be able to see the forms normally now.
