we currently use a form and the form looks pretty good on-line but the output of the filled in data from the form when it gets submitted to us is just the parsed raw data not in a nice form like we would like. The raw data doesn't have the questions being asked either...just the raw data. Can we see how your form(s) look when they get submitted?
we need a form that goes back to the customer that shows their answers to the form and we need a nice completed form for our records.

By default the headers, text fields are not included in the notification emails. You'll be able to add only the fields from the form to the notifications. However, the email composer is a full editor that allows to you to insert graphics, create tables, etc. The headers or text on your JotForm can be copied and pasted here in the email content.
You may also want to refer to the following guide that the submission can be included with the text and headers.
So that you can place a PDF link on your email notification as well. This can be done by Setting Up an Email Notification, and under email tab, add the PDF link to your email.
To send a copy to the user filling out the form, create an autoresponder:
