I'm also experiencing the same problem. I cleared both the form and browser cache however I am not having any success in accessing my forms.

I switched back to using version 3 and I was able to edit my form again. After playing with this a few times and switching back from version 3 to 4 I found the problem to only occur when version 4 is being utilized. Perhaps there is still a few kinks that need to be worked out with this new version.

I was about to reply mentioning that I was able to successfully open your form in edit mode, and provide a fallback v3 link in case it somehow still doesn't work for you, but I see that you figured that out already :)
