This is the 3rd or 4th time I have emailed regarding an issue.

I have requested this account be cancelled and we are still being charged for the monthly fee.
The initial request was dated January 9, 2017 and I have sent 2 follow-up emails since with no response.
Please CANCEL this account immediately and refund my payment of $19.99 USD to the credit card provided.
Please respond to firstname.lastname@example.org as the above email has been cancelled.

I have checked your account is Free. My colleage has cancelled your Bronze subscription and refunded the last payment.
