Why are my all forms disabled?

When I open my form using link it show That 

                 This form is disabled.

I request to jotform team to please take the probleam very seriously..  

I see that the account is auto-suspended by our anti-phishing system which is strict to scan each form to calculate the probability of phishing activities. It is not allowed to create login forms or collect sensitive information like Credit card numbers, etc through JotForm. Such forms are considered as phishing forms and associated accounts are automatically suspended. Please refer to our Terms of usage for more information on phishing. 

I see that you have created a form with Login which is not allowed. It is required to accept our terms and delete the guilty fields or forms from your account in order to activate your account. Please confirm so that we can delete the form and activate your account.
