A few questions about DocuSign.
Using 4.0. I have removed and added a DocuSign Widget several times and have a Docusign account. Form only displays "please run the widget."
Is it possible to insert more than one DocuSign fields into a single form?
What is the difference between modal and popup display?

I tested your form and I wasn't able to replicate this problem, I added the widget to your form and it worked for me:
Please try again and if the issue persists try Clearing your Browser's cache.
It is not possible to insert more than one DocuSign widget to the form.
The difference between modal and popup is that popup will open a popup screen where your customer will continue with signing and modal will open within the same screen.
