I have used Jotform to create a registration form for a summer camp for last two years and it's been successful except for one complaint. Some of the groups that come to the camp would like the option of registering their whole group all at the same time and processing one single payment for the entire group. For example, if a group of 50 wants to register, how can one person enter all of the required info and then make one payment?

Dynamic fields would be the best tool that may fit on your requirements. Registrants can easily add more fields dynamically on the form as they pleased. Here are some dynamic fields that you may want to try.

You can also apply hide/show conditions on your form if you'd like to hide/show fields depending on the chapters selected.
Here is a sample form you can check and clone using show hide conditions

Hope this helps.
