My form has been disabled, can you help me?

    Hi

     

    My form has been disabled too ! May you help me? I've just finished it but I may not use because it's disabled !

    The problem is that you are using a form to request login credentials and this is prohibited by our terms.

    Hmmm ok... May I enter in my account to delete my form and build another one? This time I'll not put login... Thank you for the response :)

    My form is doing the same, but with my work address: tommy.bussino@cardinalfinancial.com

    For some reason, when I put the email in the field above, it just sits in a "please wait" after hitting "post answer"
