    Local instance will no longer allow for documents to be uploaded (except IE)

    We are running a local instance of JotForm on our server. Approximately 2 weeks ago the ability to upload documents and images stopped working. However,  it seems to work in IE?? We have tried Chrome, FF, Safari, Opera and they all come back with an error meesage: 

    There are incomplete required fields. Please complete them.

    There are no fields that are blank and the field the software highlights is the document/image upload field. We have tried this with jpg, doc, docx, pdf etc.. The message on the media upload is this:  

    ! You can only upload following files: pdf, doc, docx, xls, csv, txt, rtf, html, zip, mp3, wma, mpg, flv, avi, jpg, jpeg, png, gif

    Any assistance would be greatly appreciated.
