"Other" option formatting does not match rest of form

Several of the questions on my form need to utilize the "Other" option, so I selected it. However, the checkbox and the text box do not match the formatting on the rest of my form, and I cannot figure out how to change it. 

I reviewed your form and I was able to see the formatting issue with the Checkbox field "other" option.

To fix this, try resetting first the form Themes to use the Default theme.

I was able to fix it by just resetting on my test form.
