How to have a summary of the form fields data?

dear folks

for this form, the users will input information

for example that in the first section, pediatric caregivers.

i would like to know how to use JOTFORM so the information in these fields is presented as a "summary" in the section "summary report"

i have explored the possibility of using update/calculate field, but am thinking this is too "clunky"

you expertise is always eagerly awaited by me
