Why I'm unable to download uploaded files?

Some of the submissions for my form looks like there is an attachment attached but when I try to open it. i get an error message that attachment not found.

If could be that attachment was not fully uploaded so the file is not complete. A good way to avoid such problems is to enable multiple uploads in your form so your users can see the progress of the uploaded file and see that the file is actually uploaded:
