How do I create a review form where customers can give a 1 to 5 star rating?

    I would like to add a review form to my website.  It should allow people to click on the number of stars they want to rate our company (like Facebook or Google use).

    Do you offer that functionality on your forms?

    You can use the Survey Elements group of fields to add the rating field on your form.
