We use jot form to allow parents to register their children for trips. Some parents register their children for multiple trips. Is it possible to populate a form with that family's information once they enter their e-mail address i.e. a condition states that if a particular email address is entered, contact details are automatically filled in from the spreadsheet.

It is not possible to prepopulate form with data from a spreadsheet.

You can use Conditional Logic Inserts Text/Calculation Into A Field to achieve this, but it will work only with text box fields.

Here is my demo form: https://form.jotform.com/70533228047957

Enter email@test.com to email field to test it.

You can also clone my form and inspect conditions that I have set.

Other possible way would be to prepopulate your form and prove you users prepopulated link that already has their data. You can create prepopulated link of each of your users.
