Form freezes when filling it out

I am getting a number of messages from recipients of my form all saying the same thing - the latest being: "We are experiencing an error when filling the travel questionnaire form online. The form freezes and we are unable to proceed." Is there anything we can do?

I have checked your last edited form 70184027912351, named "Oaktree Traveler Feedback Form", and it seems to be working fine. I have also made a test submission, which went through without any issues:
