Prepopulate app is not working

The app doesn't appear to be working. I clicked the "Choose Form" button, selected my form, and clicked "Continue." The app did not proceed to the next step. Please advise.

I was able to replicate mentioned issue.

I will escalate this issue to our developers. We will inform you via this thread once this issue is resolved.
