Form not working

    I have been developing the form -

    after making recent changes I  am unable to access the form as I get the message

     

    "Form not found This form is disabled."

    Help please.

    Your account got suspended because of the word "password" with our anti-phishing filter tool. I checked your account and you're asking for Facebook Password which violates our term of use.

    I have removed the guilty fields and activated your account so you can use and manage your forms properly.
