Why are my forms disabled?

    My forms are disabled, seemingly for no reason. I haven't reached the limits yet of a free account and they say enabled in settings. 

    Does anyone know how to resolve this?

    Your account was auto-suspended by our Anti-Phishing Tool. That's the reason why your forms were disabled. I checked your form (http://www.jotform.us/form/70567313305148) and I noticed that there is a field labeled "Username" on the form. Please note that asking such details from the users is against our Terms of Use.

    Phishing. You agree that Company may terminate your JotForm Service immediately if a form's purpose is found to be deceptively obtaining: sensitive credit card information, social security numbers, user login credentials, or other sensitive personal information. All these activities are considered as Phishing and any account along with the violating forms will be suspended immediately.

    I have removed now the said field and re-activated your account. Please refrain from doing this in order for your account not to suspended again.
