How to show a new field based on a previous answer.

    Hi Team,

     

    I wanted to create a form to send quotation to customers.

    Form must have multiple level data entry. 

    For example : When I chose Birthday event drop down, set of "services" choices must display.

    When I select one of the choice like "decoration" there must be another choices list must display .

    When I chose "Theme 1" decoration. Next level I must chose "Package" . So totally four levels.

    Similarly to Decoration I want to entertainment choice, Media coverage choices, Furniture choice wtc..

    Can some one help me to build from you team ?

    I have the mock up.
