How do i add a video to a form?

    What is supported can i upload a video downloaded directly from my laptop/phone or will i need to upload it to youtube first?

    If you want to add a video to your form, video will have to by uploaded somewhere. You can upload it to youtube and then add YouTube widget to your form. 

    You can also use Vimeo widget. 

    You can also get source URL of your video and add it inside of iFrame embed widget.

    Third way is to get embed code of your video and add it inside of Text field.
